New Patient Appointment

The new patient appointment is the first step in beginning your treatment at NOVA IVF. This is an opportunity for you to sit down with Dr. Schmidt and review your prior medical records and learn about treatment options that are available to you. It also gives you the chance to see how we are uniquely different from some of the larger institutions in delivering our care.

It is extremely critical that Dr. Schmidt and his staff learn what is important to you as you consider starting a treatment that may have a major impact on the rest of your life.

We are proud of the type of care we deliver in the field of fertility treatment and we are honored that our patients have opened up this very personal part of their lives and have entrusted us to deliver that care.

Dr. Schmidt is completely focused on the patients that he is currently treating. To deliver this high level of care he chooses not to have a large volume of patients which unfortunately can sometimes extend the wait time for the new patient consultation. Once a patient has their new patient consultation with Dr. Schmidt they can begin treatment with their very next menstrual period without delay!

Please use the following links to complete the “Comprehensive History Form” and “Registration Form” prior to your first visit at NOVA. If you need a release of your medical records, complete the “Medical Release Form”. Once completed, you can print and mail the forms to Nova or save the documents and e-mail the forms to Nova. Please note – sending e-mails from your personal e-mail account may not be privacy protected and/or encrypted.
