- If you have not done so already, please complete the Fertility Questionnaire And Registration Form.
- If you had any prior infertility tests or treatments, we would like to review your medical records prior to your initial appointment.
Please contact the medical facility(s) where you were treated and have your medical records forwarded to us.
Alternatively, you can print out the Medical Record Release Form (PDF), fill it out and mail or fax it to the location of your prior treatment.
- If you have had a hysterosalpingogram (X-ray dye study of the Fallopian tubes, HSG), we would like to review the actual X-ray films.
Please mail a medical release form copy directly to the radiology department where the HSG was performed.
- Your initial consultation will last approximately one to two hours. If applicable, both partners are encouraged to be present.
To reduce the treatment cost and to minimize delays, a brief physical examination should be included in your initial consultation;
otherwise you can have your physical on a subsequent visit.

Air purity is a crucial factor in the success of an Advanced Reproductive Treatments laboratory. Due to ongoing treatment cycles at Nova IVF,
we ask that all patients and their partners refrain from wearing perfume/cologne, body powder or scented body lotions when coming to the clinic.
- It is possible, but quite unlikely, that we may need to change the date or the time of your initial consultation at Nova IVF with little advanced notice.
This can happen if your appointment falls on a day when the physicians are required to perform a number of oocyte retrievals.
The timing of the oocyte retrieval procedures depends on each individual's ovarian response and we have less than 48 hours notice prior to any retrieval procedure.
Sometimes the appointment can be rescheduled for a later time the very same day.
If we do need to reschedule your appointment, we will do our utmost to find the earliest available appointment date for you.
- In consideration of our other patients, we respectfully request that you refrain from bringing children with you for your
initial consultation and subsequent visits to the clinic.
- Nova physicians are teaching professors. Third-year residents in Obstetrics and Gynecology do their reproductive medicine rotation at Nova.
A resident might be present during your consultation and subsequent visits to listen and observe only.
Please let us know if you prefer not to have a resident present. Your treatment at Nova IVF will not be different based on your decision.
